When your agents act, is a person on watch?

A named person stands watch over each lane of agent action. Responsibility is assigned before the agent runs. A team owns the system. Responsibility is real, but no one is named to the action. No one is specifically on watch. Ownership surfaces after something breaks.

Q2 of 5 · THE FLOOR

What stands between an agent and an irreversible act?

A deterministic floor. The act is blocked at mechanism until a person commits it. A policy layer. The agent is instructed not to, and usually does not. Reach. If the agent can call it, the agent can do it.

Q3 of 5 · FAIL CLOSED

On error, timeout, or low confidence, what does the agent do?

It stops and hands the watch to a person. It depends on the path. Some flows stop, others retry or proceed. It keeps moving: retries, guesses, proceeds.

Q4 of 5 · THE RECORD

After an agent acts, what does the record show?

An append‑only record: who held the watch, what was proposed, who committed it. Corrected by amendment, never deleted. Application logs under shared credentials. Reconstructable with effort. Final state only. The reasoning is gone.

When a session ends or a model changes, how is the watch relieved?

A formal relief. State is closed and handed off, and the next watch reads it before acting. Informally. Context carries in documents or in heads. It is not. Each session starts blind.
